When you book an appointment, contact us, or place an order, you provide information directly: your name, phone number, email address, and, where relevant to scheduling or care, details such as date of birth, insurance information, and a description of your symptoms or health history. Our online booking is handled through Jane App, our scheduling platform, and orders placed through our store collect shipping and billing details.
Payment information. Store payments are processed by WooPayments (powered by Stripe). Card numbers are entered directly into the payment processor’s secure system; we do not store full card numbers on our servers.
Information collected automatically. Like most websites, we use cookies and a standard analytics tool (Google Analytics via Google Tag Manager) to understand which pages are visited, which devices and browsers are used, and where visitors come from. This helps us improve the site. Essential cookies are required for the site to function; analytics cookies help us measure and improve content.
How We Use Your Information
- To schedule appointments and communicate with you about your visit (including reminders and follow-up).
- To provide and coordinate your care, including clinical notes kept in your secure patient record.
- To process, fulfill, and ship store orders and handle refunds.
- To answer questions you send us and to improve the content and performance of this website.
Third Parties We Work With
We share information only with the service providers needed to operate the clinic and this website: Jane App (online booking and scheduling), Stripe / WooPayments (payment processing), Google (website analytics), and our hosting provider (which stores this website). We do not sell your personal information, and we do not share your health information for advertising.
Health Information, An Important Note
Our website forms are for scheduling and general questions, they are not a secure patient portal and are not intended for urgent medical concerns. Please do not submit symptoms that need urgent attention through this site; call us or seek emergency care. Health information you share with us is used for your care and is kept in your confidential patient record, handled under our HIPAA notice of privacy practices. Cookies and Your Choices

We keep appointment and order records as long as needed for billing, legal, and care-coordination purposes, and clinical records for the retention period required by Arizona law and professional standards. Website analytics data is retained by Google on its standard retention settings.
Your Rights
On request, we will tell you what personal information we hold about you, correct inaccuracies, or delete information where we are not required by law to keep it. We do not knowingly collect personal information from children online; for pediatric patients, a parent or guardian provides information directly to the clinic.
